Partial Hospitalization Program Overview

The Jackson Park Hospital Partial Hospitalization Program (PHP) provides treatment to individuals with acute and persistent psychiatric disorders who require coordinated, intensive and comprehensive multidisciplinary treatment.

The hospital’s PHP serves as an option for individuals to continue an intensive level of care after an inpatient hospitalization. PHP serves as an alternative to inpatient hospitalization when an individual is experiencing acute symptoms of mental illness, but does not require 24 hour care and in instances when outpatient care has been determined to be insufficient and/or ineffective.

The Partial Hospitalization Program is designed to help individuals suffering from psychiatric conditions resulting from:

  • Depression
  • Anxiety
  • Mood, Personality and Thought Disorders
  • Relationship / Family Issues
  • Life Transitions/ Aging Process Issues
  • Drug Abuse

The program is geared towards individuals who present no imminent danger to themselves or others and who are able to sustain themselves in the community (residing either at home with family or in a residential facility).  The program’s mission and vision is the stabilization of symptoms while assisting the individuals in developing the skills necessary to enhance their quality of life.  The program is based on the belief that individuals can make significant changes in their behavior in dealing with life’s stresses and challenges.

Program Services

All participants are evaluated upon entering the program and an individualized treatment plan is formulated. A Case Manager is then assigned who oversees a multi-disciplinary team consisting of Registered Nurses, Licensed Clinical Social Workers, Mental Health Counselors and Activity Therapists in providing the treatment set forth in the plan. The treatment plan is carried out under the supervision of a Board Certified Psychiatrist who takes an active participatory role in both the planning and coordination of the patients care.

  • Therapeutic Community which Encourages Individual Involvement, Responsibility, & Improvement in Roll Functioning
  • Discharge/Aftercare Planning

For dually diagnosed participants (clients with substance abuse and behavioral problems), the PHP offers specific programming to address substance abuse issues and minimize relapse potential in addition to addressing their psychiatric issues.

Program Offerings

Jackson Park Hospital offers two Partial Hospitalization Programs Monday through Friday, except holidays.  The morning PHP starts at 9:30 a.m. and afternoon/evening PHP begins at 3:00 p.m.  The PHP provides approximately 5 hours of structured group treatments that allow for breaks between each session and meals tailored to the time of day.

PHP Referral Process

Referrals to the PHP program can be made by a physician, mental-health professional, a hospital or intermediate care facility, friends, family and oneself.  Call the PHP Intake Department at (773) 947-7630 for more information or a free confidential assessment.
