Outpatient Services

Working with your doctor, The Diabetes Education Center team, comprised of experienced dietitians, diabetes educators, and registered nurses, creates a care plan just for you that includes a wide range of services such as:

  • Education Classes and Counseling
    • Blood sugar monitoring
    • Insulin self-management skills
    • Oral medication self-management skills
    • Diabetes seminars
  • Support for you and your family
  • Proper Foot Care Techniques
  • Exercise counseling
  • Stress management
  • Diabetes counseling during pregnancy
  • Nutrition and diet classes and counseling and individual assessments

Inpatient Services

Whether you are hospitalized for a diabetic condition or another ailment, your diabetic team will work with your physician while you are an inpatient at Jackson Park Hospital.

Referral Services

If you do not have a primary care physician, you may choose one of our Board Certified physicians. When appropriate, your doctor may refer you to a specialist or other physicians.

You can fax your blood sugar levels, diet, and exercise results to your Diabetes Educator. Also, you can call to schedule classes, to ask questions about your diabetes care or to get more information about our program and services.
