PATIENT ADVOCACY

Jackson Park Hospital provides Patient Representatives as a service to our patients.  Their responsibility is to the patient.  Should you have any issues or concerns while at the hospital, a Patient Representative will help find the right person to assist you.

The Patient Representative

  • Serves as a representative among administration, physicians and nurses.
  • Helps when you or a family member has concerns regarding medical care, treatment or services provided at the hospital.
  • Functions as a neutral party to investigate problems or help find solutions.

To contact your Patient Representative, call (773) 947-7744 or talk to the Department Manager.  If you have further concerns you may dial zero (0) and ask the operator to call the Administrator-On-Call.
