YOUR HEALTH …YOUR DECISION

At Jackson Park Hospital, we honor your right to make your health care wishes known.  The Illinois Living Will Act and the Power of Attorney for Health Care Law were passed to give you a voice in your medical treatment if you are unable to speak for yourself.  These laws allow you to refuse death delaying care and to designate someone to make health care decisions it you are incapable of doing so.
