Access Community Health Network is a health care organization that
currently operates forty-seven federally qualified community health
centers (FQHC) in the Chicagoland area. The mission of Access Community
Health Network is to provide high quality, cost effective, safe,
comprehensive, primary and preventive health care in underserved
Chicagoland communities. Access at Jackson Park was recently established
as a FQHC on 09/11/06.
FQHC's are nonprofit, consumer-directed corporations that provide
comprehensive primary health care to the underserved, underinsured and
non-insured individuals. These groups include migrant workers and
non-citizen visitors and guests in the United States. FQHC's provide
services to all persons regardless of their ability to pay and charge for
services on a community board approved sliding-fee-scale that is based on
patient’s family income and size. We also accept all forms of insurance
except HMO's.
Hours of operation of Access @ Jackson Park are:
8:30 am – 5:00 pm, Monday – Friday
8:30 am – 12:30 pm, Saturday
