HIV Care Program

The Care Center at Jackson Park Hospital (JPH) is an initiative from the Foundation of Jackson Park Hospital, which was founded in 2004.

The Care Center serves the uninsured and insured HIV positive patients.

Our philosophy is patient centered. We accept each patient’s individuality and embrace his/her strengths as well as opportunities. Without judgment, the team addresses the patient’s most pressing need and works to resolve the issue. We endeavor to meet the needs of each patient holistically. Frequently, life circumstances contribute to struggles with poverty, addiction, depression, anxiety and self-esteem. The Care Center opens the door to improved health, refined awareness, self-confidence, the ability to make healthy choices and empowerment.

The Care Center at Jackson Park Hospital (JPH) provides compassion and hope for its patients. The Care Center is a multidisciplinary team of qualified professionals consisting of physicians, nurses, social workers and other clinicians. This program has proven to be a reliable asset to the community, for we continue to uphold an open door policy. Our office is always open for community residents and hospital staff to receive information and referrals for their needs. Since inception, the Care Center has an outstanding record of participating in community event and health fairs, in which HIV/Hepatitis testing, education, resources, as well as, STD presentations are provided.

We provide screening, evaluation, education, counseling, palliative and supportive services and clinical management to the patients living with HIV/AIDS by utilizing the skills and expertise of our primary care physicians, specialists, clinical psychiatrists, treatment-adherence counselors, Ryan White case managers, social workers, nutritionists and pharmacists, all under one roof.

With the collaboration of the Chicago Department of Public Health and Gilead Pharmaceuticals, the Care Center has established an Emergency Department HIV Testing program. Free, confidential, non-invasive (oral) HIV testing is offered in the Emergency Department 24 hours a day. This program has proven to be an effective way to locate HIV+ individuals who are lost to care or who are unaware of their status.
